How Our Geriatric Care Managers Support You:

• Comprehensive Assessments: evaluate the client’s physical, cognitive, emotional, and environmental needs to identify areas requiring support. 
• Collaborative Care Planning: Actively listen to the client and family, observing their unique circumstances to develop a personalized plan of care. 
• Home Safety and Elopement Review: Conduct a thorough assessment of the home environment, providing recommendations for hardware and technology solutions to enhance safety and security. 
• Health and Medication Management: Implement a review of medical conditions and coordinate prescription setup and monitoring, including a pharmacist-led evaluation of medications and supplements to ensure safe use and adherence to prescribed regimens. 
• Daily Living Support: Develop structured schedules for hygiene, exercise, and engaging activities tailored to the client’s preferences and needs. 

Ongoing Follow-Up: Reassess progress regularly and update care plan accordingly to ensure the highest level of client satisfaction and effectiveness of care.  

We provide as much or as little support as you need. Our home care services range from occasional 4 hour shifts to 24/7 care and include the following:

Our services ensure safe passage from hospital to home by coordinating care, managing medications, arranging follow ups, and providing support for a smooth transition.

• Collaborate with hospital discharge planner to facilitate rapid discharge to rehab or home 
• Continuity of care through hospital, rehab, and home 
• Coordination with inpatient OT/PT to train caregivers on client-specific exercises to continue progress at home 
• Coordination with home health professionals to adjust support as their services phase out
• Drastically reduced risk of future hospitalizations
